The "no re-sim policy" was set in place with these two decisions in S17 week 1:

Essentially we found significant errors with the Austin Copperheads and chose to re-sim week 1. After this, we found errors with many teams.

If you are referring to "game breaking error" I was just using the phrase Bex used.
